• 2 Dental Cleanings with Doctor Exam and Consultation (Adult/Child Prophylaxis.
• X-Rays (Panorex, Full Mouth Series, Bitewings or Periapical).
• Oral Evaluation with Hygiene Instructions.
• Fluoride as needed (no age limit).
• Oral Cancer Screening.
• Up to 2 Emergency Exams with X-Rays.
• All other procedures receive a 15% discount from our standard fee.
• When paying for dental procedures with Care Credit you will receive a 5% discount.
• Membership fees are to be paid in full with cash, check or credit cards (VISA, MasterCard, Discover, or American Express) at the time of enrollment.

How it Works

• This program is a discount plan, not a dental insurance plan.
• This plan cannot be used in conjunction with another dental insurance plan.
• The plan participant is responsible for utilizing the plan's services. Non-used services are non-refundable.
• After evaluation, if additional dental treatment is needed, written financial arrangements are required before scheduling an appointment.
• This plan only applies to services rendered at Midlothian Dental Center and does not include any treatment or services provided elsewhere by any other provider.
